Please remember that school provision at this time is the absolute last resort for families.
Why this form? By knowing in advance we can minimise the number of staff we need on site, in an attempt to keep them safe and reduce the risk of spreading COVID-19.
Please ensure your requests are up to date prior to 6pm on the day before provision.
